If you have dental coverage, please bring all pertinent information such as your benefit card and the booklet that your employer may have given you detailing your coverage. We will be happy to help you interpret your coverage and to call your insurance company for further clarification if needed. We will do our best to ensure that all your forms are properly filled so that you can be reimbursed quickly. Although we will try our best to guide you through making claims, it is your responsibility to understand your coverage and be responsible for any charges that may not be covered.
